We will NOT sell the Polish purchased using this Fund for at least 12 months from the date of purchase. If sold, we agree to repay the Fund for the full amount of the award credit used.

We give parental consent for a post about the Award to be placed on the special Facebook page and/or website page dedicated to Marty's Memorial Fund: (must select one option)

A) We consent to use of the first name of the youth and a photo of purchased Polish, however, we do not consent to the use of the youth's last name nor to the use of images of this youth.

B) We consent to include the first and last name of the youth recipient, as well as photos pre-approved by their parent/guardian.

We will provide the required 11 monthly updates, as per the Fund's Guidelines.
